Not only are we caregivers to the patient, we are also there for the family and loved ones. Grace Hospice social workers are an integral part of our team. They are professionally trained in helping patients and their family with the social, psychological, financial and family problems that can occur when a terminal illness is diagnosed. These are just a few of the social services we provide:

  • Information on community resources
  • Arrange “respite” care
  • Linkage to our Extended Care Facilities and private duty
  • Information regarding Advanced Directives, Living Wills, DNRs and DPOAs
  • Funeral arrangements and alternative services
  • Referrals
  • Crisis intervention
  • Discharge planning

There is no out of pocket expense for these services.
